GECS Reablement

The Global Edge Care Services (GECS) provides reablement services to you as step-down services following a hospital stay, but they can also be step-up services, aiming to provide the necessary support to prevent any further deterioration in your health that could lead to a hospital stay.

Reablement services are time-limited, often to periods of six to twelve weeks. However, given the importance of personalising support to achieve your own goals and aims, contracting and commissioning these services is most effectively done not on the basis of time periods and tasks, but on the outcomes desired for you to be able to live as independently as possible following an accident, or illness and resulting stay in hospital.

The role of a carer in a Customer’s reablement can be a delicate one.

Your friends and family are not always aware of the reablement ethos, of maximising independence and may want to step in and do things for you. We therefore encourage carers to help them to change their own behaviour to support you to be more independent.

Step 01

Customer’s goals are initially set by the referrer. This could be a member of the hospital or Community Team.

Step 2

GECS’s service introducer reviews goals and makes an assessment on a scale of 1-5 on how near they are to reaching these goals. Support Workers draw on goal setting tools

Step 3

The Support Worker works with the Customer to enable them to achieve their goals and sends
in electronic observations. Goals sheets are left in the Customer’s home

Step 4

The Customer completes their period of reablement.

When a reablement package of care is coming to an end it doesn’t have to mean a your care support finishes. Should you feel the need to continue a level of care to help you feel more confident in your own home we are always there to help. Just talk to your career they will be able to tell you the services that are available and the best course of action to take to suit your new care requirement.
